*I approve that the amount listed be charged to my credit card and be applied as a deposit towards my complete course fees. I will be invoiced for the remainder of the course fees and any additional charges that may arise. One form must be submitted for each registrant. If I choose to pay only the deposit at this time, I understand that the remainder is due on/or before December 05, 2023. Failure to pay when due may result in forfeiture of my deposit and course attendance. A NONREFUNDABLE fee of $500.00 is required to reserve my attendance to the 12th Annual High Altitude Comprehensive Implant Symposium. Additional information will be sent after registration has been accepted.
Additional Hotel Nights:
We have negotiated special rates for 3 days prior to the course beginning and 3 days after the course ends. All hotel nights are the sole responsibility of the attendee and his/her guests and are NOT part of any offered package. Contact MEISINGER USA for information on how to make your hotel reservations.
Important Notice:
- Course speakers and course sponsors are subject to change
- Rooms are limited at The Hythe Vail to the first 100 registrants
- Should the course be canceled by MEISINGER due to unforeseen reasons, all monies collected prior to the event will be promptly refunded
- Acts beyond MEISINGER’s control that may preclude this course from being held shall not be grounds for a refund
- All registrations must be made through MEISINGER USA
Cancellation Policy:
Conference Registration: Full conference registration refunds, less a $500 non-refundable fee, will be granted if written requests are received by Meisinger USA on or before 5 p.m. MST on November 30, 2023. Cancellation requests received after the November 30 deadline will not be eligible for credit or refund. Please submit refund requests by emailing [email protected].
Hands on Participation Course Cancellation: Full course registration refunds will be granted if written requests are received by Meisinger USA on or before 5 p.m. MST on November 30, 2023. Cancellation requests received less than 30 days prior to the first official day of the Scientific Session, will not be eligible for a credit or refund. Please email requests to [email protected].
All cancellations and refunds will be processed within 60 days after HACIS 2024.
Review Course: All attendees who cancel their course registration will be charged a $500 non-refundable fee automatically. Attendees who do not contact Meisinger USA and cancel their registration by the date of the course will forfeit their entire course fee. Exceptions to the cancellation policy (emergencies, illness, etc.) will be considered on an individual basis and may require documentation for verification
CANCELLATION NOTICE FROM THE HYTHE VAIL RESORT: Cancellation policies will be provided to the attendee by the Hythe Vail Resort, if requested, prior to their booking. Please note, after November 20, 2023 hotel reservations are non-refundable.
