Description: Molar with insufficient composite filling occlusal and distal with marginal discoloration, occlusal cover filling after trepanation*, caries extension into the outer third of the dentin, sufficient root filling with cover filling (4 canals), enamel crack in transverse fissure between the palatal cusps extending running occlusally demineralized enamel around the cavity margin
